CenClear, a provider of mental health and substance use disorder (SUD) services is seeking a dedicated and experienced Wellness Nurse to join our team. This full-time role offers an opportunity to contribute to the overall wellness and improved quality of life to the individuals we serve.

Location

1 position based at our MH/SUD outpatient facility located in Huntingdon and Yeagertown, PA. Travel to other offices, home and community as necessary.

Hours

• This is a full-time position working 40 hours per week, 52 weeks per year

Compensation

• Yearly Salary Range $44,000-$45,300

Qualifications

• Licensed Practical Nurse (LPN)
• Three years of experience in mental health services
• Must obtain the following clearances prior to hire: PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate

Responsibilities

• Participate in all required CenClear, CCBHC and wellness/health related education and trainings
• Provide wellness coaching and related trainings to staff, individuals in services and community partners
• Collaborate with staff to coordinate and promote health and wellness assessment, counseling and referrals
• Promote a culture of health and wellness through ongoing participation in meetings and community outreach
• Collaborate and coordinate with healthcare providers for continuity of care
• Facilitate referrals, as needed
• Assist in establishment of screening, assessment and tracking procedures
• Assist in program evaluation and outcomes reporting
• Collect required data for outcomes measurement and contract requirements
• Submit all necessary administrative paperwork according to procedures
• Document all wellness coaching sessions and other wellness/integration activities
• Document all contacts made with consumers and other community agencies
• Psychiatric Prescriber support one day per week
